Results of the IXth meeting of “SibNeuro” Siberian Association of Neurosurgeons.

The regular IXth meeting of the Siberian Association of Neurosurgeons was held at the premises of the Federal Neurosurgical Center (Novosibirsk) under the chairmanship of Jamil Rzaev, Head Doctor of the Center.

The meeting was dedicated to the issues of surgical treatment of  chiasmo-cellar areas tumors. An invited speaker, Dr.Bakhtiyar Pashaev, a neurosurgeon of a neurosurgical division of the Interregional clinical and diagnostic center, a member of the North-American Skull Base Society, presented 2 reports. Dr.Ekaterina Gormolisova, a neurosurgeon of the oncological division of the Federal Neurosurgical Center had also presented her report.

During the 1st report, Dr.Pashaev shared his experience of performing transnasal surgical procedures from microsurgical resection of pituitary adenomas to endoscopic surgery. Complication of transnasal procedures were presented in detail, a particular accent was made on the methods of decreasing the percentage of such complications.

Dr.Ekaterina Gormolisova told about the experience of the Federal Neurosurgical Center (Novosibirsk) in transnasal surgery of pituitary adenomas. She presented information about surgical procedures performed in the Federal Neurosurgical Center, about postoperative patients’ management and obtained results and complications.

The final report “Endonasal endoscopic skull base surgery” was presented by Dr.B.Pashaev where he stressed an anatomy of chiasmo-cellar area. He reported about the methods of plastics of skull base defects either using allogenic graft or self-tissues such as advanced flap of mucosa or pericranial graft with a pedicle.

Both lectures of Dr.Pashaev were filled with small but very significant practical suggestions brought from a clinic of Pittsburg where he took courses several times.

Besides the lectures Dr.Pashaev had performed 3 surgical procedures. The first patient was a 3-years old child with previous cerebrocranial trauma. Liquorrhea was noticed in this patient during a year. The surgical procedure was performed exactly for this reason. The defect closure was performed with mucosa graft. The patient was released from hospital in satisfactory condition without any signs of liquorrhea. The second patient was a woman with pituitary adenoma. The tumor was removed. In postoperative period normalization of somatotropin was noticed. The third patient had non-functioning pituitary adenoma. This case is an interesting one because the patient had abnormal tumor expansion: it grew intracellarily and reached basilar sinus and had a huge fragment in nasal pharynx. Both patients with adenomas were released from the hospital in satisfactory condition without additional neurological deficit and anhornomia.

Besides the participants of the meeting being in the conference-hall, specialists from St.-Petersburg, Kemerovo, Krasnoyarsk and Irkutsk took part in the meeting with the help of real-time translation.

After a vivid discussion the chairman of the meeting announced preliminary agenda of the Xth meeting of the Association that will be held on August 28, 2015 in Irkutsk.
