On April 24, 2015, the regular meeting of the Siberian Association of Neurosurgeons was held at the Federal Neurosurgical Center (Novosibirsk).

Jamil Rzaev, the Head Doctor of the Federal Neurosurgical Center traditionally became the chairman of the meeting.

Andrei Y. Grigoriev, MD, professor of neurosurgery and neuroreanimation of Moscow State University of Medicine and Dentistry named after A.I. Evdokimov, Head of Neurosurgical department of the Federal Endocrinology Research Center (Moscow), Evgeniy V. Sivov, neurosurgeon of Regional Clinical Oncology Center (Khabarovsk) and Anton V. Kalinowski, neurosurgeon, PhD, Head of the operating block of FNC (Novosibirsk) shared the results of their own research at the meeting.


On a lecture entitled "Surgical treatment of hormonally active pituitary adenomas" Professor Grigoriev Andrei detailed questions of treatment of pituitary adenomas. Particular attention was paid to the hormonally active pituitary adenomas. It was told about the methods of diagnosis, additional research methods and modern methods of treatment. Emphasis was placed on the ability of both conservative and surgical treatment. It provided clear criteria for selection of patients for a particular method. Also scheme of post-operative assessment of the hormonal status of patients were presented.

Dr. Anton Kalinowski in his report "Features of surgical treatment of meningiomas of the skull with intraorbital spread" spoken about the results of surgical treatment of 13 patients with meningiomas of the skull base, growing into orbit, operated in the Federal Neurosurgical Center in the period of 2013 – 2015 years. The complexity of the surgical treatment in this group of patients is in the need for expanding access to the base of the anterior and middle cranial fossae, with resection of bone structures forming the walls of the orbit. This increases the radicality of surgery. When doctor covered a variety of orbit accesses, their advantages and disadvantages, methods of reconstruction of the orbit using autografts.

In conclusion, Dr. Evgeniy V. Sivov spoke on "Stereotactic radiotherapy of intracranial brain tumors". The report reflected current issues of stereotactic radiosurgery in the treatment of brain tumors. At the beginning of the message were presented historical information about the development of radiosurgery techniques. It has been stated about the possibilities of modern radiological methods of treatment, recommendations for patients selection and criteria for determining the specific method of exposure. Also, results of radiosurgical treatment of patients with various tumors of the brain were presented.

In addition to the conference during the VI session of "SibNeuro" surgeries involving Andrey Grigoriev were demonstrated.

Two patients were selected for demonstration: one patient with spontaneous CFS leak and patient with recurrent pituitary somatotropinoma.

The complexity of the first case was the localization of the bone defect, and the presence of cerebral herniation to the sinuses. It was made the repair of the defect with special adhesive compositions. Postoperatively CFS leak was treated.

The second patient was treated with relapse of hormonally active pituitary adenoma. In this case, the work of neurosurgeons was complicated by the fact that it was a second operation, and the normal anatomy has been changed. There were scarring of the nasal area and sellar region that presents a difficulty in conducting such operations. The use of endoscopic equipment and optics allowed to visualize tumor tissue and remove it completely.

Both patients in the first postoperative day were activated and discharged 6 days later without neurological deficits and hormonal disorders.

The next meeting will be on May 29, 2015, invited lector is Sergey Novikov, Head of Radiation Oncology and Nuclear Medicine department Oncology Research Institute named after N.N. Petrov (St. Petersburg). He will make a presentation on "Methodological and clinical aspects of stereotactic radiotherapy of brain tumors". In the near future program of the event will be shown on the website of "SibNeuro".
