“Functional Neurosurgery-2014” annual educational course.

Efficient operation of any medical institution is impossible without professional doctors. Medicine is developing quite rapidly, more and more new methods and technologies for the treatment of certain diseases appear every year, so that doctors should keep his eye on the ball and be aware of current events in the world of medicine.

There is no doubt that constant self-training plays an important role in this process, but do not underestimate the social events - various conventions, conferences, exhibitions and other events organized to gather together experts of a particular field of medicine to discuss topical issues and problems.

Such events give doctors a chance to share experiences with colleagues from different medical institutions not only from our country but also from all around the world. Doctors also can make useful contacts, which further lead to a long and fruitful collaboration.

In this section, we will cover upcoming and past events, the organizers or the participants of which are doctors of our Center. And reviews about participation in various activities will be available to read.

We hope that this information will be interesting for our colleagues and will be useful in their future work. We will be glad to see you, colleagues, on medical events!

Welcome speech of Dr Jamil Rzaev

On October 30-31, 2014 “Functional Neurosurgery” annual educational course took place in the Federal Neurosurgical Center.

Neurosurgeons and specialists of related professions from Moscow, St.-Petersburg, Novosibirsk, and Yekaterinburg presented their reports on topical issues of functional neurosurgery. The educational course consisted of 4 sections (epilepsy, movement disorders, pain syndromes, follow-up care in neurosurgery) and 3 online surgical procedures (amygdalohyppocampectomy, DREZ procedure, cervical neurostimulation).

Krylov, Scientific Director of the Center (Academician of the Russian Academy of Medical Sciences, Doctor of Medicine, Professor, Honored Scientist of the Russian Federation, Laureate of State Prize of the Russian Federation, Head of the scientific division of N.V.Sklifosovsky Scientific and Research Institute of First Aid, The Member of the Russian Neurosurgeons Association board, The Member of European and World Associations of Neurosurgeons, The Member of the Supreme Attestation Commission, editor-in-chief of “Neurosurgery” journal, Deputy Chairman of Moscow Neurosurgeons Society) opened the educational course.

The first section was dedicated to drug-resistant epilepsy. In her report  Alla B.Gekht (honored doctor of the Russian Federation, Doctor of Medicine, Professor of neurological and neurosurgical department of the Russian National Research Medical University named after N.I.Pirogov, Moscow) has demonstrated the concept of drug-resistance and the main principles of anti-epileptic drugs prescription.

The next report was dedicated to morphological MR-characteristics of small epileptogenic zones in focal forms of epilepsy. Darya V.Rudenko (M.D., doctor of MRI diagnostics of Medical and Diagnostic Center of the International Institute of Biological Systems, St.-Petersburg) illustrated MR characteristics of the most commonly omitted pathologies such as focal cortical dysplasia, heterotopy, hippocampus sclerosis, abnormal development of cerebral hemispheres that can be the reason of drug-resistant epilepsy. The optimal protocol of MR-testing for more complete visualization of epileptogenic sites was presented.

The report performed by Iosif V.Volkov (neurologist-epileptologist, MD, the head of epileptologic center of St.Valentine based in the “SibNeuroMed” City neurological center, Novosibirsk) was focused on EEG-testing. Examples of electroencephalograms of patients with different types of pathology were given in the presentation; the main diagnostic EEG characteristics of different types of epilepsy were emphasized.

Prof. Aleksandr Shershever

In the report based on the vast experience of epilepsy treatment in children, Aleksandr S.Shershever (Doctor of Medicine, Professor of nervous diseases and neurosurgery department of “Ural State Medical Academy”, the head of organizational department of Neurooncological center of Sverdlovsk region, Yekaterinburg, neurosurgeon) showed a score of clinical cases of successfully performed temporal lobe resections, callosotomy, hemespherectomy. 

Igor S.Trifonov (neurosurgeon, teaching assistant of neurosurgical and neuro-intensive care department of Moscow State University of Medicine and Dentistry named after A.I.Evdokimov, Moscow) told in details about modern methods of diagnostics and surgical treatment of epilepsy. In the end of lecture section the clinical case of drug-resistant epilepsy with dextral mesial temporal sclerosis was presented. There was a broadcasting from the OR where Dmitry Y.Rogov, a neurosurgeon, performed amygdalohyppocampectomy under neurophysiological control.

The second section was focused on movement disorders. Elena A.Khabarova (neurologist, specialist in Parkinson’s disease of the functional division of the Federal neurosurgical Center, Novosibirsk) presented her report on criteria of choice of patients with extrapyramidal pathology for surgical treatment. The presentation contained video of patients with dystonia and Parkinson’s disease in “on” and “off” periods that helped illustrating criteria of “good” candidate for surgical treatment.

More detailed information about cervical dystonia and deep brain stimulation in extrapyramidal disorders was provided in the report of Aleksey A.Tomskiy (M.D., research scientist of functional neurosurgery group of the Academician N.N.Burdenko Research Institute of Neurosurgery, Moscow).

The report of Victoria V.Vardosanidze (neurologist, honored doctor of the Russian Federation, the head of extrapyramidal diseases center, Novosibirsk) was devoted to organization of help for patients with extrapyramidal pathology in Novosibirsk region. Doctor Vardosanidze told the audience about the founding of the “School for patients with Parkinson’s disease and their relatives”. Patients and their relatives are offered meetings and work with psychologist, physical therapist, they listen to special lectures. Besides, patients and their relatives visit theaters and museums of the city.

The second day of the educational course was dedicated to pain syndromes and follow-up care. Olga B.Doronina (neurologist, M.D., assistant professor of neurology of Novosibirsk State Medical University, Novosibirsk) told about the schemes of medical treatment of neuropathic pain.

In his report about neurostimulation of spinal cord for treatment of chronic pain syndromes Dmitry Y.Rogov (neurosurgeon of the functional division of the Federal neurosurgical center, Novosibirsk) defined the main indications and contra-indications, operating technique and possible complications in the process of constant epidural neurostimulator implantation.

Prof. Oleg Dreval

Emil D.Isagulyan (neurosurgeon, algologist, M.D., research scientist of the Academician N.N.Burdenko Research Institute of Neurosurgery, Moscow) told about the first Russian experience of motor cortex stimulation in deafferentational pain syndromes. 

A very detailed report on brachiplex avulsion and technique of DREZ procedures was presented by Oleg N.Dreval (professor, Doctor of Medicine, Chairman of the department of the Russian Medical Academy of Post-Graduate Education, Moscow). Just after the lecture Doctor Dreval together with the doctors of spinal division of the Federal neurosurgical center performed DREZ procedure.

Andrey I.Holyavin (M.D., senior research scientist of the laboratory of stereotactic methods, teaching assistant of neurosurgical department of Military Medical Academy, St.-Petersburg) made a report about the effect of combined intervention in drug-resistant psychic disorders.

One more report was devoted to facial pain surgery. Jamil A.Rzaev (neurosurgeon, M.D., Head Doctor of the Federal neurosurgical center, Novosibirsk) presented an algorithm of surgical treatment of different types of facial pains. Doctor Rzaev also told about first results of facial pain treatment in the Federal neurosurgical center in Novosibirsk. In the end of his report Dr.Rzaev presented a clinical case of a patient with occipital headaches. There was a demonstration from the OR where Dr.Isagulyan performed test occipital stimulation.

The section devoted to surgical aspects of follow-up care in neurosurgery was opened by the report of Vladimir M.Kenis (head of foot abnormality division, neuro orthopedics and general diseases, associated professor, senior research scientist of Scientific and Research Institute of Children’s Orthopedics named after G.I.Turner, St.-Petersburg) on “surgical treatment of spasticity in children: the choice of tactics and the role of selective dorsal rhizotomy”.  Dr.Kenis also mentioned about intrathecal baclofen and botulinus toxin injections for correction of great tonicity in children with cerebral palsy.

Andrey V.Kozlov (surgeon, M.D., associate professor of neurosurgical department of Novosibirsk State  Medical Academy, head doctor of “Zolotoye Sechenie” clinic, Novosibirsk) presented a report on comprehensive treatment of functional and trophic disorders of peripheral nervous system.  In his report Dr.Kozlov discovered the main principles of reconstructive and surgical repair in cases of extensive limbs’ injuries. The final report about facial nerve Reinnervation was presented by Dr. Aleksandr B.Dmitryev (MD, neurosurgeon of oncological division of the Federal Neurosurgical Center).

At the end of the educational course the participants discussed interesting moments and plans for the nearest future.
