Results of the “Training of microvascular anastomosis” practical course

“Training of Microvascular anastomosis” practical course was held during April 9-11, 2018 at the premises of the Federal Neurosurgical Center (Novosibirsk).

Dr.Andrey V.Dubovoy, Dr.Konstantin S.Ovsyannikov and Dr.Dmitry M.Galaktionov were the faculties of the course.

The course started from the welcome words of the head of neurovascular division, Dr.Andrey V.Dubovoy.

Eugeniya Ilyinova opened the theoretical part of the course and told the audience about suture materials used for different types of vascular sutures with different types of needles. She also told about the rules of correct suture materials choice for a particular surgical case.

After that Konstantin S.Ovsyannikov presented his report on “Techniques and main types of microsurgical anastomosis”. He told about the types of vascular anastomosis used in the neurosurgical practice (end-to-end, end-to-side, side-to-side), techniques of surgical knots in anastomosis (women’s knot, marine knot, surgical knot), rules and consequences of making knots in creation of vascular anastomosis in neurosurgical practice.

Andrey V.Dubovoy presented a lecture on “Surgical brain revascularization” where he highlighted historical aspects of anastomosis in neurosurgical practice, indication for revascular surgery, methods of patients’ examination for brain revascularization, techniques, details and special aspects of anastomosis creation in cases of different vascular brain diseases, possible complications and ways of their elimination.

The lecture “Review of topical hemostatic agents used in neurosurgical practice” was presented by Anastasiya Safronova. She told the audience about hemostatic materials used in neurosurgical practice, about the ways of some hemostatic agents’ application in cases of vascular brain surgery.

Practical part of the course included training of professional skills. Participants trained technique of making knots on thick silicon tubes 5/6 mm; anastomosis of “end-to-end” type – on thick silicon tubes 6/5 mm; anastomosis of “end-to-end” on thin silicon tubes 0.5/1 mm.

Participants also trained technique of farming of microvascular anastomosis of “end-to-side” and “end-to-end” types on chicken’s femoral artery with the help of a microscope and microsurgical instruments. Diameter of chicken’s femoral artery was 1-2 mm that is compatible with the diameter of brain vessels. Thus, it’s like an imitation of brain revascularization surgery - EICMA.

 

During the last day of the practical course pre-operational planning of mini-approach for extra-intracranial microanastomosis with the help of “LINS Makhaon: doctor’s working station” was demonstrated to the participants.

Participants could also watch video of surgical procedure of extra-intracranial microanastomosis creation with some comments.

Then participants had a chance to watch “side-to-side” anastomosis creation on aorta and cava performed by Dr.Andrey V.Dubovoy.

The guests from Moscow, St.-Petersburg. Cheboksari, Novosibirsk and Rostov-on-Don were among participants of the course.

All the participants were awarded with certificates and they got 12 points.
